Returning Learners to SelfDesign’s Online Learning Program
This form is for learners who:
- Are re-enrolling for the 2025/2026 learning year.
- Are currently enrolled in the SelfDesign Learning Community for the 2024/2025 school year and working with an educator.
- Are siblings of learners who are currently enrolled or registered in 2024/2025 who do not have an IEP or require Inclusive Education services.
Important: If your new sibling learner has an Individual Education Plan (IEP) or may require Inclusive Education support you must speak with our Intake Coordinator before submitting an application.

Proof of legal residency (must be in the Primary Parent/Guardian’s name, showing physical address, and be dated within the last 90 days. Acceptable documents include:
- Utility bill
- Drivers licence
- Credit card invoice
- Rental agreement (all pages)
- Mortgage statement
- British Columbia ID
- Municipal tax bill
- Notary authorization letter
For New Learners (or if returning learners’ details have changed):
- BC Medical Card number
- Proof of legal citizenship
- If born in Canada: Long form birth certificate (showing parent names, scanned or photographed, ready to upload)
- If not born in Canada: Non-expired proof of legal citizenship, such as:
- Permanent resident form/card
- Immigration Canada permit
- Immigration Canada Visa
- Canada Immigration Record of Landing Form
Additional Documentation (if applicable)
- Name of the last school attended for each learner
- Proof of custody
- Legal name changes
- Indigenous Ancestry

New Learners to SelfDesign’s Online Learning Program
This form is for learners who:
- Are enrolling to our online learning program, working with a BC-Certified teacher, for the upcoming 2025/26 learning year
- Do not have an IEP or require additional Inclusive Education support.
Important: If your new learner has an Individual Education Plan (IEP) or may require Inclusive Education support, please fill out the Expression of Interest form by clicking on the New Learner button in the Inclusive Education section of the enrolment page.

New Learners to SelfDesign’s Inclusive Education Online Learning Program
This form is for learners who:
- Have had an Individual Education Plan (IEP) previously, or
- Would benefit from special education support, or
- Have had a BC Ministry of Education designation based on a previous assessment
